MREC-09 · MILITARY RECORDS LIBRARY
Medical, Clinical, Injury & Disability Records
MREC-09: Medical evidence from baseline health through treatment, hospitalization, occupational exposure, injury, duty limitations, line-of-duty issues, disability evaluation, and later healthcare/benefit claims.
What this record can establish
Diagnoses and treatment; dates and facilities; reported symptoms; objective findings; limitations; medical-board evidence; treatment chronology and later medical opinions.
What it does not establish automatically
A treatment record may document what was reported without independently proving cause, duty status, legal responsibility, or service connection. A diagnosis is not itself a benefit decision.
Preservation package
Entrance and periodic exams; clinical/hospital records; imaging/labs; profiles; injury reports; LOD documents; MEB/PEB files; VA exams; release authorizations and record requests.

- Institutional purpose: Understand why the record was created and what it was designed to record.
- Corroboration: Seek independent records; copies and derivatives from one source do not create independence.
