Use current official forms, preserve evidence and filing proof, and calendar every deadline and effective date.
Before using a form
- Confirm the form number and exact purpose.
- Confirm the current edition or revision date from the official repository.
- Check whether the transaction has moved online or uses a different service-specific process.
- Read the form instructions, signature rules, supporting-document requirements, and filing address/portal.
- Confirm whether the deadline is based on mailing, receipt, electronic submission, decision date, separation date, or another triggering event.
Evidence packet
- Form as filed
- All attachments and exhibits
- Source records supporting each material fact
- Cover letter or statement identifying requested relief
- Proof of mailing, fax, upload, electronic receipt, or in-person delivery
- Case, tracking, or confirmation number
- Calendar entry for expected response and review deadline
Do not silently replace an old form
If an older form appears in a prior file, preserve it as historical evidence. Use the official current form for a new transaction unless the controlling instructions say otherwise. Record which version was used and when.
